What information is required for the injured worker's details?

You need to provide the injured worker's family name, first name, company, position, and the time of the injury or illness. Additionally, you must describe the nature of the injury or illness and specify the bodily location affected, as well as the exact location where the injury occurred.

Who needs to complete and sign off on the injury report?

The person making the entry, typically the manager or supervisor of the injured worker, must complete the report. They need to sign off, confirming that the report is accurate. The report must also include whether an investigation was conducted and what controls were implemented to prevent a recurrence of the incident.

What details should be included regarding the injury incident?

You must describe how the injury or illness was sustained and indicate whether any equipment was involved. If equipment was involved, provide specific details. Additionally, list any witnesses to the incident, including their full names and contact numbers.
